How can I adjust the report format to more like the format you have in the example?

I'm not sure what you mean? There are several things that you can change. The first is if you can create an nice "graphic" header, that really makes it look professional. We recommend starting with a rectangle that is roughly 1 inch high and 8 inches wide. Then fill it with you logo, company name and other info.
All the invoice settings are in Setup -> Invoicing. You can turn fields Off or On.
